Lifetime Wellness’ medical insurance is effective the first of the month following an employee’s full-time start date. We offer a consumer choice option, giving our employees an opportunity to select the level of benefits that will best suit their needs. The PPO network within our plan is one of the largest in Texas, making it easy to access necessary medical care.
Life Insurance (Optional)
Lifetime Wellness provides the opportunity for full-time employees to purchase a $15,000 basic life and AD& D insurance policy. This policy is effective the first of the month following an employee’s full-time hire date. Additional coverage is also available for employees who want to increase their amount at an affordable group rate.
401(K) Retirement Plan
Colleagues can reduce taxable income by saving and investing in Lifetime Wellness’ 401(k) plan. Employees can enroll the first of the month, following 60 days of employment. This great benefit is available to our PRN and part-time employees, as well as full-time.
LIfetime Wellness’ dental insurance plan is cost effective for our employees and provides superior coverage. Most preventative care is available without a deductible and paid 100% by the plan. Major services and orthodontics (for children up to age 19) are also covered.
Vision insurance available to Lifetime Wellness employees is also cost effective and provides excellent coverage. Most office visits require minimal co-pay, and the plan provides a frame allowance, as well as contact lens coverage.
Short and Long Term Disability Insurance (Optional)
Lifetime Wellness provides the opportunity for all full-time employees to purchase a short term and long-term disability plan. Our short term disability insurance is one of the few that provides 24/7 coverage.
Paid Time Off & Paid Personal Days
Scheduled time away from professional responsibilities is well-earned and part of any individual’s well-rounded development. As such, Lifetime Wellness provides colleagues with a Paid Time Off (PTO) benefit.
